The Spanish flu? This may not have had the highest cfr but I think the totals were higher. Upwards of 50 million to 200 million people world-wide. I don't think there were that many people in the entire world during the other pandemics.

Spanish flu, definitely, in term of absolute number of individuals that died.
If - instead of absolute value, one takes into account the relative impact on global population, the ''Black Death' perhaps it would be the worst.

My guess would be H1N1(1918) for highest total although that pandemics' numbers are very uncertain. I would not discount HIV as it has already claimed 35 million lives, which is within the 1918 fatality estimates, and is not done yet. The one I am most glad to have avoided is the Black Death. As already pointed out the global population increase has largely limited the answer to later events as in 1347 the population was about half a billion, 1918 1.5 and now 8. Also for earlier events poorer communications would have limited geographic spread with many communites disconected from the global trade network.
