Uganda: 436 Being Monitored for Marburg Signs

The Ministry of Health and experts from the Centre for Disease Control and Prevention have set up a field diagnostic laboratory in Kabale district to quicken testing of suspected Marburg cases.

It will take three hours, instead of 24 hours, for patients to get their results at the new facility based at Kabale Regional Referral Hospital.

Health minister Dr. Christine Ondoa said the facility would also serve neighbouring districts while further serological results are to be undertaken at the Uganda Virus Research Institute in Entebbe...

Ministry of health hires 10 foreign experts to handle Marburg outbreak

Ministry of Health and its partners have hired ten technical experts to handle and contain the spread of Marburg cases in the identified districts.
The ministry of health says the death toll of both the probable and confirmed cases stands at eight, with the latest being a case that died at the isolation facility at Rushoroza Health Centre III on October 27.
The Minister Dr Christine Ondoa clarified that since the onset of the outbreak, her ministry has collected a total of 45 samples of which nine were confirmed positive; five in Kabale, two in Kampala and two from Ibanda.

She says that working closely with the US Center for Disease Control and Prevention (CDC), whereby the ministry has set up a field diagnostic laboratory at Kabale Regional Referral Hospital. ?All samples from the affected neighbouring districts will hence be taken to this laboratory for quick diagnostics. This will shorten the time when we get results to three hours from the original 24 hours due to distance. Further serological testing will be undertaken at the Uganda Virus Research Institute (UVRI),? Ondoa said.

Dr Ondoa says due to the presence of cases in other districts, the ministry of health has established temporary isolation facilities to accommodate the suspected and confirmed cases. In Ibanda, a temporary isolation ward has been created at Ibanda Hospital, while plans are underway to set up a proper isolation facility by tomorrow.
She says at Mbarara Regional Referral Hospital, a separate temporary has been designated for he suspect Marburg cases. A triage has also been set up at the causality ward.
We have assembled a team of experts to work in the newly established isolation facilities and they are expected in these districts today. We also plan to undertake infection control procedures in these facilities as safety measures for the workers and the admitted patients.

Dr Ondoa says there are seven suspect cases (student nurses) quarantined at Ibanda. These cases attended to the confirmed case that later died at Mbarara Regional Referral Hospital on October 24. Other health workers who attended to the patient are closely being monitored.

Dr Ondoa dispelled rumours that, ?Dr. Sheila Ndyanabangi, the head of the Mental Health Unit Division, had contracted Marburg and had been isolated. Dr. Ndyanabangi has not been isolated but has been advised to exercise social distancing. She is one of the contacts who are being monitored. She has not developed any signs or symptoms of the disease and therefore cannot be isolated from the community. 12 People Admitted Over Marburg as 436 are Put Under Surveillance



436 people have now been under surveillance over the marburg fever which broke out in the country at the beginning of the month.
The 436 people come from different parts of the country including Mbarara, Kampala, Ibanda, Fort Portal, Jinja, among others.
In addition, there are up to twelve people who were admitted to different hospital after having signs of the fever. Of the twelve, two are in Mulago hospital, eight at Bisolowoza and two at Mayanja Memorial Hospital in Mbarara.
The ministry of health has called out to the people to be careful as the fever spreads randomly across the country.
The marburg fever was first reported in Kabale after a group of people in one family, four in number, died from the disease. By then, it had not been identified as marburg and several people attended the burial without any protective measures.
However, after the burial, it was identified as such and several people were put under surveillance but were not isolated. Kabale district has already banned public gatherings as the fever takes root in the country.
The fever broke out after the country was declared Ebola free. Marburg haemorrhagic fever in Uganda ? update (29 October 2012)

31 October 2012 - As of 29 October 2012, a total of 19 cases and 9 deaths have been reported from 5 districts namely Kabale district, in south-western Uganda, Kampala (the capital city), Ibanda, Mbarara and Kabarole. The case fatality rate is 47.4%. The outbreak was declared by the Ministry of Health in Uganda on 19 October 2012. One of the 9 deaths reported is a health worker.

Currently, 13 patients have been admitted to hospital (2 in Kampala, 8 in Kabale, 3 in Ibanda) and their contacts are listed for daily follow up. The latest confirmed case was admitted to Ibanda district isolation ward on 26 October 2012.

Blood samples from 9 cases have been confirmed for Marburg virus at the Uganda Virus Research Institute (UVRI).

The World Health Organization (WHO) and international partners including, the Centers for Disease Control and Prevention (CDC), the Uganda Red Cross (URCS), African Field Epidemiology Network (AFENET) and M?decins-Sans-Fronti?res (MSF) are supporting the national authorities in outbreak investigation and response.
The national task force has identified additional health care workers and epidemiologists to strengthen the teams in the field. Training of health workers on infection prevention and control, surveillance and clinical case management is ongoing. Social mobilization is ongoing including the dissemination of IEC (Information Education Communication) material, sensitization on Marburg preventions and control and broadcast of information through radio channels. The first shipment of personal protective equipment (PPE) provided by WHO arrived over the weekend.

The WHO Regional office has deployed an epidemiologist and a logistician to Uganda to support the response teams on the ground. In addition, a social mobilization expert from WHO Zambia Country Office and a logistician from the Regional Rapid Response Team network have been mobilized for immediate deployment. More experts are being identified by the Global Outbreak Alert and Response Network (GOARN).

As the investigation into the outbreak continues, WHO and partners continue to support the national authorities as needed in the areas of coordination, infection prevention and control, surveillance, epidemiology, public information and social mobilization, anthropological analysis and logistics for outbreak response.

Neighbouring countries have been contacted to strengthen cross border surveillance and preparedness to prevent cross border spread of the outbreak.

WHO advises that there is no need for any restrictions on travel or trade with Uganda.

Marburg isolation unit set up in Fort Portal

A Marburg isolation centre has been set up at Fort Portal Regional Referral Hospital following the death of a suspected Marburg victim.
The centre was set up on Thursday.

On Wednesday, Alisen Murorwa, a resident of Kiceri Village in Kasenda Sub-county, Kabarole District, died with symptoms similar to those of a Marburg virus.

He was vomiting blood and bleeding through the eyes and nose. Murorwa died in the general ward at the referral hospital causing panic among the health workers and patients.

Last week, Murorwa?s daughter, Mbabazi Amooti, 35, also died under unclear circumstances.
However, health workers are not sure if she had Marburg symptoms, since she did not die from the hospital.

Murorwa was buried on Thursday by health workers, despite resistance from his relatives and friends.

The District Health Officer, Dr Richard Mugahi, said the tests of the deceased were taken to the Uganda Virus Research Institute (UVRI) in Entebbe and they expected the results on Friday evening.

He said: ?Health workers are still investigating if the deceased travelled to Kabale and Ibanda, where there has been an outbreak of Marburg.?
Health workers are also monitoring the friends and relatives of Murorwa and his wife who could have come into contact with him and that should the results turn out to be positive, they will be quarantined.

Meanwhile, authorities in Luweero District have set up a surveillance team at the District Medical Department to monitor the health situation at Kikube Village, Luweero Sub-county after a seven-year-old boy died on Wednesday shortly after complaining of fever and vomiting, which according to health officials, are some of the symptoms of the deadly Marburg virus.

The family of the deceased and residents of Kikube Village were ordered not to handle the body for burial and had to wait for more than 12 hours before a medical team from the UVRI arrived to take the specimen sample for testing and conduct the burial of the deceased. What Is the Current Situation?

The Ugandan Ministry of Health (MOH) has reported an outbreak of Marburg hemorrhagic fever in the Kitumba subcounty of the Kabale and Ibanda Districts in southwestern Uganda. As of October 28, 18 cases and 9 deaths have been reported. Nine of the cases are laboratory confirmed.

The city of Kabale is within several miles of the outbreak. Kabale is often a stopping point for people visiting both Lake Bunyonyi and the Mgahinga and Bwindi Impenetrable National Parks, which are famous for mountain gorilla tracking. This city is also a transportation hub, with roads leading to both Rwanda and the Democratic Republic of the Congo.
CDC is working closely with the Ugandan MOH and the World Health Organization (WHO) to control the outbreak. CDC does not recommend any travel restrictions at this time...

The deadly Ebola and Marburg diseases, which hit Uganda recently, are negatively impacting on the country's tourism.

A number of foreign tourists have reportedly cancelled their trips to Uganda due to fear of the disease.
The country's Ministry of Tourism has resolved to cull all wild animals in national parks, as they suspect these could be carrying the fatal viruses.
Mid this year, there was an Ebola outbreak, where 14 people died. A Marburg virus outbreak, a few weeks ago, has so far claimed 10 lives.
Tourism Minister, Maria Mutagamba said her ministry was working with the Health Ministry to carry out research and identify animals carrying the viruses so they could adopt preventive measures.
Ebola and Marburg diseases are believed to originate from wild animals, and as Ugandans are fond of game meat, they are prone to infection from the deadly diseases.
"We are looking at several ways of fighting the deadly diseases. Fencing the game parks so that local people do not access them would be the best option but it is very expensive.
"We shall eliminate animals suspected to be carrying viruses of Ebola and Marburg," Mutagamba said.
She said because of Ebola and Marburg diseases a number of foreign tourists cancelled their trips to Uganda, leading to loss of income.
A medical expert, Dr Asuman Lukwago said suspected cases of Marburg had been found in monkeys, chimpanzees and bats.
He said when some people poach wild animals in game parks, they end up killing and eating those infected with deadly virus and they also get infected.


But Uganda wildlife authority spokesperson, Lillian Nsubuga wonders how animals would be identified for the cull, as there are thousands of them.


"I wonder how they will identify animals with Ebola or Marburg virus. There are thousands of buffaloes and hippos. How will they check them for the virus?" she asked.
She blamed people living near the national parks for poaching uncertified and wild animals for game.
