Samples have not tested positive for H1N1, but the CMO seems to suggest that this is because of a mutation. There also seems to be concurrent outbreaks of malaria, dengue and typhoid, but the feeling I get from this report is that the mysterious fever which is killing hundreds may be flu.

These are the numbers of confirmed H1N1 cases and fatalities from Uttar Pradesh since August 1.





It can be seen that a post monsoon outbreak occurred peaking around Sep 5. In total, 283 cases and 20 fatalities were reported.

There is little doubt that these numbers are a gross underestimate. Consider the United States for example - from May 2009 to April 2010 around 320 pediatric flu fatalities were reported. But the CDC estimates that around 1,270 children actually died as a result of the pandemic. In other words, in a country where pediatric reporting is mandatory, where presumably each case had access to hospital care, only a quarter of the estimated fatalities were actually confirmed. One wonders what the ratio would be for rural Uttar Pradesh? 1:10? 1:20? If so, then the confirmed numbers may represent at a minimum 200+ fatalities for this H1N1 outbreak in Uttar Pradesh. In the US the cfr is estimated around 0.02% - even if the cfr in India were greater, say 0.05%, 200 fatalities would represent 400,000 infected individuals.

Alternatively, if 10% of the population became infected, we would expect 3,800 fatalities using the estimated cfr from the US.

As the outbreak peaked in the first half of September, people who are currently suffering from respiratory symptoms may be suffering from co-infections and may not be testing positive for H1N1. It is also possible that if nasal swabs are being tested, that any lingering deep lung infection is being missed. I do not believe that a mutation theory is necessary to account for negative tests at this stage.

Uttar Pradesh has a population of 190,000,000. One would expect 10's of thousands of flu deaths from the State each year. In India as a whole, a child dies each minute from pneumonia - 10,000 per week, and that's just the kids. While the current outbreak in Uttar Pradesh is of interest and bears watching for any continued increase in fatalities - I believe that it is explicable within the context of seasonal post-monsoon outbreaks.
