H5N1 in Tenth Swan Highlights Surveillance Flaws in England

The confirmation of H5N1 in a tenth mute swan in Dorset raises serious surveillance concerns. Although DEFRA continues to claim that H5N1 in wild birds in England is limited and has not spread, the basis of their statement is negative results from a fatally flawed surveillance program.

H5N1 in dead mute swans in the swannery in Dorset was reported over a month ago in mute swans that were collected as early as December 27, 2007 (see satellite
map) . The first two swans were euthanized because they were near death, and all subsequent positives have been from dead mute swans. The most recent positive swan was collected February 4, 2008. Thus, although H5N1 has been in the region for well over a month, the surveillance program has yet to detect H5N1 in a live healthy mute swan, and has failed to detect H5N1 in any other species.

H5N1 levels in mute swans are higher than other species, which may explain the failure to detect H5N1 in other species. Similarly, the failure to detect the Qinghai strain (clade 2.2) in live wild birds is fairly common because the levels are lower. Thus, the surveillance failures are not limited to the DEFRA program.

However, these major limitations are ignored in DEFRA reports, which continue to claim that there has been no evidence of spread. This lack of evidence is due to the insensitive assay, not to the absence of spread.

The recent DEFRA report described the identity levels in the H5N1 from the first four positive swans. The differences between the isolates indicated the introductions into the swan population were independent, and not due to a common source. This genetic diversity supports a varied reservoir in the mute swan population, or repeated infections from a local source. However, neither has been identified in the surveillance program, even though it has been in the ”enhanced” mode since H5N1 was first detected, in early January.

The failure to detect H5N1 in live birds or any wild birds other than the mute swans. signals flawed surveillance. H5N1 is readily transmitted to a wide range of avian species which would have contact with H5N1 infected birds in and around the swannery. However, these H5N1 infected birds continue to circulate undetected.

The failures should be acknowledged by DEFRA, and the misleading statements about “lack of evidence” should cease.

Executive Summary
1. Highly Pathogenic Avian Influenza (HPAI) was confirmed in Great Britain on
10 January 2008, when tests had shown that virus strain type H5N1 (HPAI
H5N1), was present in the carcases of three wild mute swans (Cygnus olor).
These carcases were collected on 7 January 2008 from the Fleet Reserve on
the Dorset coast, and examined as part of Great Britain?s avian influenza wild
bird surveillance programme.
2. HPAI H5N1 was later detected in carcases of three further mute swans
collected from the same area. No infection has been identified in any other
individual or species of wild bird in Great Britain since April 2006, when an
infected whooper swan (Cygnus cygnus) was found dead, washed up in
Cellardyke harbour in Scotland.
3. Analysis of the virus indicates that it is related to contemporary viruses from continental Europe and is most closely related to a cluster of isolates
recovered in mid to late 2007 from wild and domestic birds in the Czech
Republic, Romania and Poland.
4. Expert ornithological advice indicated that swans, or other wild birds,
particularly wildfowl from the same population, would move within the Fleet,
between the two Royal Society for the Protection of Birds (RSPB) reserves at
Lodmoor and Radipole and up to 3km inland, and thus potentially place
domestic poultry at risk of infection within this area. This led to the delineation of the Wild Bird Control Area (WBCA).
5. Wild bird surveillance has been increased in the area and apart from the
swans reported above, no other evidence of avian influenza infection has
been found in the wild bird populations.
6. This is a region of relatively few domestic poultry, and surveillance activities focussed on the domestic poultry within the WCBA are complete. No evidence of spread of HPAI H5N1 to the domestic poultry population has been found.
7. The source of infection for the swans has not been identified, however the
most plausible hypothesis is that it was introduced by one of the migratory
wild birds that make up part of the population on the Fleet during the winter.
8. The Health Protection Agency have been informed of the incident and have
provided advice on the potential human health risk, which is not considered
further in this report.

DEFRA is well aware of the data. They should release the sequences so the mistaken analysis can be brought to their attention by multiple independent observers.

The only data on the H5N1 outbreaks in the past few years was the turkey data showing the relationship between the Bernard Mathews turkeys in England and H5N1 in Hungary.

The data from the recent outbreaks in England (Suffolk and Dorset) has been withheld (as has much of the data on H5N1 in Europe in 2005 and early 2006).

Unfortunately, this is how they operate.

Many (including myself) would describe these reports as political propaganda - no science required).

Weymouth Vet virology labs, they are hardly amateurs, Niman. That they don't care to share their bird isolates with private interests, that is their business.

The report wasn't all that forthcoming, but it answered the questions I put to them.

The molecular data wouldn't be all that helpful in understanding the outbreak along Chesil Beach. It's an outlier, Niman. The fact that these point infections haven't spread throughout this large, well-established historical colony and into nearby area wetlands and poultry farms is a clue.

DEFRA has indicated that the strains from infected birds were closely related to those isolated in wildfowl and domestic poultry in Central Europe, consistent with known mute swan movement to SW England. The suggestion that infected birds had flown to this area of relatively mild, sunny weather and assured food supply, to escape frigid Siberian cold air mass that has caused much grief to eastern Europe and Central Asia in the last month, seems quite plausible.

That would deftly explain the sudden increase in the number of local positives detected in January along Fleet Lagoon and Chesil Beach, eh?

Suggestions are easy to make, especially in the absence of data. However, the report gave per cent homology of the first 4 isolates. With a sample size that small, calling any one an "outlier" is pretty tricky. The homology range is wide, especially for such a small number, and will likely be wider when all ten positives are analyzed.

The latest positive was NOT along Fleet Lagoon and Chesil Beach, and positives have been detected for well over a month now.

The data does NOT support a single introduction.

I agree the Weybridge knows exactly what it is doing, which is why the data is being withheld.

They spun a similar yarn in the winter of 2006, when birds were "migrating west in search of open water" during a harsh winter. It's a good story, but not supported by the sequence data, which identifies regional markers in the west not found in isolates from eastern Europe.

These tall tales are viable in the absence of data, which is another reason the data is withheld.

Prior to the recent positives, the only positive wild bird DEFRA could come up with was a whooper swan on the shores of Scotland (while a closely related H5N1 was being found in MUCH higher numbers in wild birds in northern Germany, Denmark, and Sweden), and although that was about 2 years ago, DEFRA has only recently released partial sequence data (3 of the 8 gene segments).

DEFRA knows exactly what it is doing, which is becoming increasingly obvious.

No dead swans have tested positive for the lethal strain of the H5N1 virus at the reserve for three weeks.
An announcement from the Department for Environment, Food and Rural Affairs (Defra) on the lifting of bird restrictions is now expected this week.
Swannery manager John Houston said staff are crossing their fingers they will be able to open for business as normal next month.
"It's very encouraging," he said. "We're not out of the woods just yet but it hopefully it won't be too much longer now."
A 20-kilometre control zone was introduced following the bird flu outbreak in January.
Bird movements have been restricted since and nine mute swans at Abbotsbury have tested positive for the disease.
Mr Houston said the last was on January 29 - meaning the swannery could soon be in the clear.
He said: "No dead swans have tested positive for a while now. Obviously swans have died in the interim but in the winter that's normal.
"Less swans have actually died this February because it's been so warm. In that respect it's been a very good month."
Mr Houston said Defra could consider lifting restrictions now 21 days have passed since the last positive test.
"We'll have to see what their take on it is," he said. "Because that timescale isn't set in stone.
"It's looking reasonably positive now and we're crossing our fingers.
"But we could still get another swan testing positive. That would put the whole process back again."
Mr Houston said swannery staff were still planning to open for the public on March 15.
He said: "It's still in the lap of the gods and it's too early to be sure but it's looking good.
"It would be tremendous if we could. It would be even better if the swans have become immune to this disease.
"Then we can all look forward with confidence and without worry."
Nicky Ives, who runs a free-range egg business at Vurlands Farm in nearby Swyre, is also hoping the outbreak is over.
She had to lock up 6,000 chickens when bird flu hit.
"We're really looking forward to the end of it all," she said.
"Now the weather's nicer the chickens are getting frantic, they're looking to get outside.
"Luckily our egg sales haven't been affected. Our customers have been very loyal."
A Defra spokesman said the situation at Abbotsbury was still being monitored.
She added: "There's been no shrinking of the restriction zones just yet but it's definitely on the cards.
"For now the restrictions are still in place but there should be an announcement this week."

Defra confirmed on 4 February that another mute swan, found in the same area, has tested positive for H5N1. This brings the total number of swans confirmed with H5N1 in the Control Area to ten. Evidence suggests that the level of infection in the mute swan population is low, and there remains no evidence of spread to other wild birds or domestic poultry.

Bird flu monitoring to merge

Defra has confirmed that from today the Wild Bird Control Area and Wild Bird Monitoring Area in Dorset will merge.
This is based on ongoing surveillance in the area, veterinary risk assessment, and expert ornithological advice about the area most at risk, in accordance with the Avian Influenza (H5N1 in Wild Birds) (England) Order 2006.
Highly pathogenic H5N1 was confirmed in three wild mute swans in the area on 10 January and to date there have been a total of 10 wild mute swans confirmed with the disease. The last case was a mute swan found dead on 29 January and confirmed on 4 February.
Acting Chief Veterinary Officer, Fred Landeg, said: "Our active surveillance and sampling in the area has provided evidence that the virus has been confined to mute swans and at a very low level. H5N1 appears to have been present in the area for about two months, during this time we have seen only a few deaths in the mute swans.
"We have no evidence of the virus being present in healthy live mute swans or in other species of wild birds. There is no evidence of disease spreading to domestic birds. On this basis we have today merged our Control Area and Monitoring Area to form a single Monitoring Area.
"It is timely to remind everyone that the UK remains at a constant low level of risk of the introduction of highly pathogenic H5N1, with a slight increase during winter and spring migration. I therefore urge all bird keepers to remain vigilant for signs of disease and take measures to isolate kept birds from wild birds."
The Wild Bird Monitoring Area must remain in place for at least 30 days following the collection of the last positive sample.

No evidence of more bird flu after 21 days

THE 21-day wait is over for the Abbotsbury Swannery as officials declare that there is no evidence of any new bird flu cases.

The Department for Environment, Food and Rural Affairs (Defra) announced that the wild bird control area and the wild bird monitoring area in Dorset will merge.

This means that poultry farmers and bird keepers living within the previous control area around Abbotsbury will be less restricted. It has been three weeks since the last swan tested positive for the lethal H5N1 virus.

Ten mute swans from the swannery tested positive for the disease in the days leading up to January 29.

This meant that bird movements were restricted by a 20km control zone introduced following the outbreak.

Acting chief veterinary officer for Defra Fred Landeg said: "H5N1 appears to have been present in the area for about two months and during this time we have seen only a few deaths in the mute swans.

"We have no evidence of this virus being present in healthy live mute swans or in other species of wild bird.

"There is also no evidence of disease spreading to domestic birds and on this basis we have today merged our control area and monitoring area to form one single monitoring area.

"It is timely to remind everyone that the country remains at a constant low level of risk of the introduction of the highly pathogenic H5N1.

He added: "The problem has not been as bad as we first thought."

The wild bird monitoring area must remain in place for at least 30 days from the last collection of the last positive sample.

Well, seems that the care is not much oriented if that's an regional, or wider source of infection, but if it have much potential to spread to wider poultry farms regions, or not.
For now, it's only busines: must the free range poultry be keept now in close, or not.
If it must, than they keep it close, if the quantity of dead wild birds was minimal, and contained (no matter if there are only swans, or other birds) they will lift off the closure.

That kind of policy keeped partly contained the poultry outbreaks in the past few years, and they will keep it that way for now.

It will be interesting to understand more precisely what will happen if they don't change that policy?
Obviously the illness will remain, but what can be done about?
For now the best policy seems to be keep the poultry in close when spreads, left at open when it deminish, and control the eventual smugling of wild birds, or infected poultry from the international commercial sources.

Exist at this moment some better way of controlling the illness globaly if the orgs gave up all the sequences, new strains vaccines excluded?

Bird flu swan close to lake
ONE of the ten swans infected with bird flu last month was found close to Radipole Lake.
A spokesman from the Department for Environment, Food and Rural Affairs (Defra) confirmed one bird was found five miles south of the other cases at Abbotsbury Swannery.
The control area and wild bird monitoring areas will now merge meaning poultry farmers and bird keepers are less restricted. Defra said the location was not significant because it was still inside the control area.

Below are public sequences in Europe which are related to the recently released clade 2.2.3 sequences from Kuwait (those with "entry not found" are other isolates from Kuwait - this list has some of the most recent deposits). Thus, the mute swan sequences in Dorset that are 99.2% identical are just other clade 2.2.3 isolates and do NOT support a common source. This type of analysis is VERY straightforward and VERY easy to interpret.

DEFRA SHOULD BE informed of its mistake.
Sequences coming out of Russia provide additional data on common source sequence. One set of sequences (all 8 gene segments) from a chicken were released last week. The outbreak was on December 15, 2007 at was at Rostov-on-Dom. Today a wild bird (starling) sequence (all 8 gene segements) is being released. I have looked at 3 so far. The HA (over 1700 posiions) and PA sequences (over 2250 positions) from the chicken and starling are 100% identical. The NA sequences (over 1300 positions each) differ by ONE nucleotide. Thus, like the chicken and whooper swan sequences from Krasnodar, the % identity will be well above 99.9%. Both of these sets are clade 2.2.3, which is present in wild birds throughout Europe, including England.

DEFRA is getting 99.2% between swan isolates, signaling independent introductions of clade 2.2.3 H5N1 from other wild birds.

I have gone through all 8 gene segements from Rostov. Three genes (PB2, NA, MP) each have ONE nucleotide difference between the chicken and starling isolates. The other 5 genes are identical. Thus, every gene has at least 99.9% identity between the Rostov chicken and starling. The samples were collected a day apart and the result parallels comparisons between a chicken and whooper swan in the Krasnodar outbreak last September.

Common sources are EASILY identified.

The swans at Dorset were NOT infected by a common source (and DEFRA knows it, press releases and official reports notwithstanding).
