The deadly Nipah virus that struck Kozhikode district in Kerala last month and claimed 16 lives in the state has been finally contained and the last of the two positive cases have fully recovered, said Kerala Health Minister KK Shailaja on Sunday.

Including Mohammed Sadik who was the first person suspected to have died of the infection, the death toll stands at 17.

The virus struck at Kozhikode which saw 13 people dying, while three deaths were reported from nearby Malappuram district, and more than 2,000 patients with fever was kept under close observation.

Cluster of 3 cases of Viral encephalitis reported
from Block Perambra, PHC- Chengaroth. The
unusual event started with death of a 26 yr
male, clinically diagnosed as viral encephalitis
at Medical College Hospital. Two weeks later 2
members of the same family reported with
similar symptoms, rapidly progressed and died
while on treatment, at a private tertiary care
hospital. On subsequent days 6 more persons
succumbed to similar symptoms, including a
staff nurse of the taluk hospital, involved in
treatment of the second case. All the affected
cases were found to have close contact with a
confirmed case/death. District RRT team along
with the central team of NCDC investigated the
outbreak. House to house active case search
and contact tracing was done. Serum and CSF
samples were sent to MCVR Manipal & NIV
Pune. Nipah Virus Encephalitis PCR test was
confirmed positive for initial three cases and
therafter many more. Isolation points
identified and infection control protocol
strengthened. Adequate quantity of PPE and
other logistics ensured in the district. Burial
protocols issued. Home quarantine for the
contacts and hospitalized observation for all
symptomatic implemented. Medical camps
conducted in the area for disease awareness and
reducing panic.

Kerala?s health care system and social capital won the battle against Nipah, but it is important to review the lessons learnt in order to prepare for future invasions. It is also important that India develop a national policy on Nipah surveillance and management.
ON May 18, 2018, Dr G. Arunkumar of the Manipal Institute of Virology confirmed what clinicians and public health officials had been suspecting: there were patients in Kerala infected with the Nipah virus. That night, Kerala declared war on an enemy we knew very little about. By June 11, the outbreak was declared over.

The index case of the outbreak reported to a subdivisional hospital on May 2. He had also been to the casualty and CT scan units and the ICU of Government Medical College, Kozhikode, before he died at the Medical College Hospital. He passed the virus on to 16 persons at these places. Once the Health Department came to realise the magnitude of the challenge, an emergency response was put in place. A control room was set up under the District Collector, and the Director of Health Services was posted there until the crisis was over. The entire health machinery was engaged to follow up and quarantine every potential source of infection. Cases were picked up as and when they reported sick and shifted safely to the treatment centre at Government Medical College, Kozhikode. These measures ensured that the situation never got out of hand. Finally, 18 persons had been infected. Of them 16 died. We had prepared for at least four times that number.

The health system and social capital of the State won the battle, even though at a cost. However, the war will continue, as there is a strong possibility of the infection recurring. Therefore, it is important to review what we have learnt and how we can prepare for future invasions.

Medical team to camp for more days
Though the Health Department authorities are yet to confirm the cause of death of two persons at Plamalakudy tribal settlement, near Adimaly, with symptoms such as fever and shivering, no new cases were reported till Thursday. A medical team is camping at the settlement, said District Medical Officer P.K. Sushama.

Mallimuthu, 40, and Pathini, 32, died on June 20 and 24 at the settlement. The cause of deaths could not be confirmed because no blood sample was available as the bodies were already cremated, Ms. Sushama said.
